Visiting Nurses are often licensed nurses (LVN’s or LPN’s) and address patients with immediate needs such as pain, wound care, and address deaths. The triage nurse provides assessment and treatment over the phone for symptom management, consult with the hospice physician, and notify the case manager or visit nurse if a visit is required. There are other caregivers involved, as well, that provide daily care with religion, daily activities of living, and keeping things organized. Home health aides (HHAs) provide personal care to patients in their home environment based on individual patient need including hygiene, comfort and housekeeping. HHAs may be hired privately by patients or their families, or provided by a Home Health or Hospice agency and are covered services under the Medicare Hospice Benefit. Chaplains are non-denominational, spiritual, and emotionally supportive professionals who are available to counsel and support hospice patients and their families. ...Describe which experiences or persons have contributed to your plans to pursue a career in a health-related field. (500 words) I have been a Registered Nurse actively practicing for over 19+ years, and I can never forget the reason I became a nurse. I am a nurse today because, I have a heart that want to serve and passionate about providing quality care to our patients especially our Geriatric population.. However, my interest in this field was sealed by my interaction with a Registered Nurse, Diane my mentor who I worked with as a Home Health Aide (HHA) in Home Health Care between 1987-1990. She observed my passion, dedication, a heart in service, and the empathetic manner, I cared for my patients who needed assistance with Activities of daily living (ADLs). She relentlessly made sure I continued to Nursing School. Diane was the kind of nursing professional, who every patient wished they had in their corner in their time of need. Her personal touch and empathetic care knows no bound. I worked closely with her in our tour of duty, and my interaction with her sealed my fate of going to school to become a nurse. I wanted to do what she does and better. We had planned biweekly joint home visit with the patient's as per Federal regulations, and also for special procedures/process or for in services for new care equipments. Through observation, interview with patients /family members she was able to finds out if there were concerns or satisfied with the care ......

...professionals and professionals in home health care and hospice(NAHC, 2012). It provides relevant, high-quality education for workers to pursue on their own time without having to be absent from work. HHAs usually work alone and must lift or turn clients who are physically incapacitated; the risk of back injury remains high. In some cases, the potential exists for infection, although the appropriate use of protective equipment can usually provide protection, if the client provides such equipment. HHAs work with people of all ages experiencing a range of medical conditions that may be painful; an HHA may need to work in a situation where a client's suffering cannot be completely relieved. Some clients are terminally ill, and HHAs may suffer emotional losses when clients die. It takes minimal education to become an HHA; some states do not require a high school diploma. Most states, however, require HHAs to hold a health professions license, which can be earned through on-the-job training experiences or through short certificate courses offered by community colleges and vocational schools. HHAs are often needed around the clock and may work full time or part time, which offers flexible scheduling options not available in other entry-level jobs. Although few opportunities for advancement exist for HHAs, the Florida Area Health Education Consortium notes that, with additional training, the profession can the first step to another health career. Enhanced Audit Tool-Hand Hygiene 1

...have the greatest staff/patient activity and interaction. This results in higher numbers of ‘Moments’ being audited in shorter time periods. | Auditing facilities where there is little staff/patient activity and interaction will result in a small number of moments being observed (i.e. non-acute, primary care, mental health settings) and resources required to undertake auditing may be better utilised measuring other aspects of a hand hygiene program (e.g product placement and availability surveys, education etc). | Currently HHA do not recommend routine HHC auditing with the 5 Moments for Hand Hygiene audit tool as an outcome measure in the non-acute, primary care or mental health setting. | All facilities should be aware of their jurisdictional requirements when planning measurements of their hand hygiene program. A number of assessment tools are available below which can be used or modified as desired. | HHC audits should only be undertaken by HHA trained and validated HHC auditors. | Annual Auditor Validation The National Hand Hygiene Initiative (NHHI) is based on the World Health Organisation (WHO) - World Alliance for Patient Safety campaign – “Clean Care is Safer Care” launched in 2005.  On the 5th May 2009 WHO relaunched their campaign as "Save Lives: Clean Your Hands" . Please click here to access the WHO hand hygiene webpage WHO have released two key Hand Hygiene documents: WHO Guidelines on Hand Hygiene in Health Care. Cervical Cancer Caused by Human Papillomavirus Infection in Latin America: a Focus on Populations in Rural Honduras

...the most prevalent cause of cancer in women in Honduras and the leading cause of cancer death for Honduran women (Ferrera et al., 1997; Garrett et al., 2013) It is critical to note that many epidemiological studies show that the human papillomavirus (HPV) DNA is detected in 80%-90% of cervical carcinomas (Ferrara et al., 1997). Vaccination again the most common types of HPV has proven successful at significantly decreasing rates of cervical cancer in developed nations. In fact, mortality rates of cervical cancer are seven times higher in Latin America and the Caribbean than in North America (PAHO et al., 2012). High prevalence of HPV infection disproportionally burdens Latin America due to in part to culturally specific sexual behaviors (HHA et al.). These behaviors include, early age at first sexual intercourse, number of sexual partners and the sexual behavior of one’s partner (Garrett et al., 2013; Almonte et al. 2008). Unfortunately, lack of access and knowledge of HPV and the HPV vaccine are also major contributors to the previously mentioned high incidence and mortality rates of cervical cancer that plague Latin America, and especially rural Honduras. In fact, in one study conducted among 632 Honduran women, only 13% had heard about the HPV vaccination prior to the study. Consequently, after the study 91% of these women stated that they intended to have their daughters vaccinated to protect them from cervical cancer and HPV infection (Perkins et al., 2011). In......

...consistently surpassed costs by higher margins than those for low-visit episodes. Conversely, episodes that comprise only three to five therapy visits have been underpaid and will be adjusted. The CMS proposal redistributes payments from high-therapy episodes to low-therapy episodes. The reference to the "plan of care" refers to the overall HHA plan of care approved by a physician. As described in regulations, the patient's function must be periodically reassessed by a qualified therapist, of the corresponding discipline for the type of therapy being provided, using a method that would include "objective measurement." Fixed Payments If a home health episode includes fewer than five home health visits, the PPS payment is replaced by a fixed payment per visit. The proposed 2012 per-visit rates are $134.25 for speech-language pathology, $123.50 for physical therapy, and $124.40 for occupational therapy (geographically adjusted). These figures represent a 1.5% increase for each discipline. The 2012 proposal also includes a revision to the 2011 requirement that a physician meet face-to-face with the patient prior to HHA admission. HHAs strongly protested this requirement; the proposal allows a physician who treated the patient in an acute or post-acute setting to inform the HHA's certifying physician of the patient's qualifying conditions. Choosing Between a Skilled Nursing Facility or Home Health Care for Your Elderly Loved Ones

...pay is used for your daily cost of living such as food, electric, phone and TV. It also helps to pay for the employees like the doctors, nurses, and administrative staff. Home Health Care (HHC) helps seniors live more independently for as long as possible. Like a SNF you can pay for this yourself which on average will cost $112.-192/per day or $3, 000.-6,000/per month. These numbers will fluctuate as it depends on the amount of services you will need. With HHC you only pay for services that you need. (www.eldercare.gov) Medicare will pay for HHC for a period of 60 days (2012 medicare.gov). The Home Health Agency (HHA) will receive one payment for the services you will receive during to 60 day period. Between the 50th and 55th day the Registered Nurse (RN) or Case Manager (CM) working with the agency you choose to provide the Home Health Aid (HHA) or Certified Nurses Asst. (C.N.A) will come to re certify you or extend your care for another 60 days if you require it. Medicaid will also cover HHC if you meet your states poverty criteria and need to receive the care as set by the states guidelines (www. medicaid.gov). The Money you pay with HHC is for services specifically for your individual needs which include Skilled Nursing (SN), housekeeping, personal care help and respite care. Comparing the cost of care The long term care costs cited below are national averages, and figures do vary by location. In urban areas elder care costs typically exceed those in......

Business Managment

...1. State the administrative agency which controls the regulation. Explain why this agency and your proposed regulation interest you (briefly). Will this proposed regulation affect you or the business in which you are working? If so, how? I chose the proposal which was posted on September 17, 2010 by the agency of HHS. HHA also known as Unites States Department of Health and Human Services. The subject of proposed regulation is “National Vaccine Injury Compensation Program: Revisions to the Vaccine Injury Table”. HHS is working to protect the health of America. I think as a person Health is the first thing you should one should take care. That is why; I wanted to choose regulations from the HHA agency. And the purpose of selecting this specific regulation is because of our new generations. It is our duty to protect the health of our children. As a parent, we should be aware of all the side effects of vaccines. We are not doctors or scientists. It is not possible for us to know the technical terms of all the effects and of vaccines. These changes on the table will help us to understand. 2. Describe the proposal/change. The Secretary of HHA is proposing to change the Vaccine Injury Table. The change will create distinct and separate listings for Hepatitis A, Trivalent Influenza, Meningococcal and Human Papillomavirus Vaccines. The Table includes a list of covered vaccines under the National Vaccine Injury Compensation Program (VICP). The VICP provides a system of......

...health facilities… and care provided by family and friends” (p. 9). There are cases that care required is for recovery and monitoring purposes that last less than 90 days, but essentially the majority of care almost always involves a series or a process of care that takes time. The interviewee department falls under this category as she works specifically with an agency where a wide range of health care services are given in their home for an illness or injury. Its goal is to treat an illness or injury. Home health care helps to make patients feel better, regain independence, and become as self-sufficient as possible. This HHA attends to monitoring illness and unstable health status, perform wound care for pressure sores or surgical wounds, provide patient and caregiver education, administer injections and intravenous medications and nutrition therapy. The HHA provide a wide array of multidisciplinary and professional as per patient need after thorough assessment. The services must be covered by Medicare or any private medical insurance. In other words, the customers must be covered by Medicare or any private medical insurance to receive care. What makes this unique than a common setting is, a home health nurse visits with in home patients and takes care of their medical need according to a treatment plan designed by the patient’s doctor. The treatment plan varies from patient to patient, depending on their condition and treatment needs. It may include collecting a......

Homeschooling vs Going to School

...to be together. Some public school kids who don’t know a lot about homeschooling always try convincing me that public school is better because of the social interaction they receive. Also, they argue that extracurricular activities will get you out of the house, and that they don’t want to be “cooped up”. Is public school the only way to “get out of the house”? Being home schooled has more opportunities than most people realize. There are home school groups all over that offer extracurricular activities, social interaction, and opportunities to get out of the house. In my home school group there are over seventy home school families that we interact with constantly, at what we call Report Day, and also at Harlingen Homeschool Academy (HHA), where you can take classes together of your choice. We even have boy basketball and cheerleading, and many other activities. So homeschooling not only has a flexible schedule and provides opportunities for your family to grow closer together, but it also can give what most teenagers want the most: interaction with others, extracurricular activities, and chances to get out of the house. Homeschooling is what you make of it. It can be very inconvenient if that’s what you want it to be, but you can make anything seem negative. You have to look at homeschooling as an opportunity, not a chore, because even though it may seem to be tedious at times, it is worth all the advantages in the end....

...increase awareness and sensitivity to our members, improve patient participation and adherence to treatment plans. Finally we will need to measure our success. According to CAHPS (2009) “The CAHPS Home Health Care Survey is a measure of patients' experiences with home health care agencies, providers, and staff. Developed by the Agency for Healthcare Research and Quality (AHRQ), this instrument focuses on patients who receive skilled home health care services from Medicare-certified Home Health Agencies (HHAs). Skilled services refer to health care services provided by nurses and therapists including physical, occupational, and speech-language therapists.” The HHCAHPS Survey instrument contains 34 items that cover topics such as access to care, communications, and interactions with the agency and with agency staff. There are two global items: one asks the patient to rate the care provided by the HHA, and the second asks the patient about his or her willingness to recommend the HHA to family and friends. The survey also contains items that ask for self-reported health status and basic demographic information (race/ethnicity, education attainment level, language spoken in the home, etc.) As stated by Home Health CAHPS (n.d.) their main objectives of Home health care CAHPS survey is to provide “comparable data on the patient’s perspective that allows objective and meaningful comparisons between home health agencies on domains that are important to consumers.
